Recurrence risk levels of prostate cancer

Oncologists may classify prostate cancer into groups based on the risk of recurrence (low risk, intermediate risk or high risk). These levels of risk can guide treatment plans for a man with prostate cancer.

Risk levelStagePSA levelGleason score

low risk (all risk factors must be at these levels)

T2a or lower

9 or less

intermediate risk (any risk factor at these levels but not higher)

T2b–T2c

7

high risk (any risk factor at these levels)

8–10

Men in the high-risk group are offered aggressive treatment. Those in the low-risk group may be offered active surveillance without immediate treatment.
